This test is available only to inpatients drawn in Royal Oak Hospital.

Specialized collection kit used, notify Royal Oak STAT Lab or Royal Oak Inpatient Phlebotomy if needed for inpatient collection.
Specimen Collection Criteria
Collect: Two Greiner Light Blue-top 3.2% Sodium Citrate tubes.
- GREINER TUBES MUST BE FILLED TO THE BLACK LINE (1/2 tube).
- Venipuncture should be performed using a 21G or larger needle (20G or 19G). Use of a butterfly 21G is acceptable.
- If drawing blood for a CBC at the same time, fill the CBC tube last.
- First collect a clearing tube (at least 2.0 mL) making sure the clearing tube does not contain any platelet inhibiting substances (e.g. EDTA).
- Clearing tube does not need to be sent to the lab.
- Fill the second and third tube (Greiner sample tubes) to the black line (1/2 tube). DO NOT UNDER FILL.
- Gently invert the Greiner tubes at least 5 times to ensure complete mixing of the contents.
- Keep in collection kit, both tubes must be HAND-DELIVERED to the Royal Oak STAT Lab immediately. Do NOT send through the pneumatic tube system.
- Discard the first 5.0 mL from an indwelling catheter to clear the line. Ensure the catheter is free of clots.
- Immediately transfer blood to both 2.0 mL Greiner sample tubes to fill volume to the black indicator line (1/2 tube). DO NOT UNDERFILL.
- If drawing blood for a CBC at the same time, fill the CBC tube last.
- Gently invert the Greiner tubes at least 5 times to ensure complete mixing of the contents.
- Keep both Greiner sample tubes in the collection kit. HAND-DELIVER to Royal Oak STAT Lab immediately.
Preparation for Courier Transport
Transport: Whole blood at room temperature (20-26°C or 68-78.8°F).
Rejection Criteria
- Specimens collected in BD light blue top 3.2% Sodium Citrate tubes.
- Specimens not collected in Greiner partial-fill vacuette tube (1/2 fill indicator) with 3.2% Sodium Citrate Blue Top.
- Specimens collected at sites other than Royal Oak Hospital campus.
- Collected specimen sent through pneumatic tube station.
- Glass tubes and 3.8% Sodium Citrate tubes.
- Specimens that are clotted or grossly hemolyzed.
- Inappropriate specimen volume, underfill or overfilled of indicator line.
- Specimens received in the Laboratory more than 4 hours past the time of collection.
- Specimen transported not at room-temperature.
- Unlabeled or mislabeled specimens Greiner-Top Sample specimens.
Inpatient Specimen Preparation
Hand-deliver both specimens in collection kit to the Royal Oak STAT Lab immediately after collection. DO NOT use the pneumatic tube system.
In-Lab Processing
Do not refrigerate or freeze specimens. Do not centrifuge. Specimens must be analyzed within 4 hours of collection.
Storage
Specimen Stability for Testing:
Room Temperature (20-26°C or 68-78.8°F): 4 hours
Refrigerated (2-8°C or 36-46°F): Unacceptable
Frozen (-20°C/-4°F or below): Unacceptable
Specimen Storage in Department Prior to Disposal:
Room Temperature (20-26 C or 68-78.8 F): 5 days
Refrigerated (2-8°C or 36-46°F): 5 days

Results available within 4 hours of receipt in the Laboratory.
Reference Range
Verify Now Aspirin assay results are reported in Aspirin Reactivity Units (ARU).
ARU values below 550 are indicative of effective aspirin inhibition of platelet activity.
ARU values of 550 or above are indicative of ineffective aspirin inhibition of platelet activity.
Test Methodology
Point of care, utilizing platelet induced aggregation turbimetric optical detection.

Limitations:
NSAID’s will interfere with this assay. Direct IIb/IIIa inhibitors will interfere and testing must not be performed within 48 hours after administration of eptifibatide (integrilin) or tirofibrin (Aggrastat), or within 14 days after administration of abciximab (ReoPro).
Clinical Utility
The Verify Now Aspirin Test
qualitative test aids in the detection of platelet dysfunction due to aspirin
ingestion. This test is not for use in patient with underlying congenital
platelet abnormalities, patients with non-aspirin induced acquired platelet
abnormalities or in patients receiving non-aspirin anti-platelet agents. (May
be used in patients treated with COX-2 inhibitors, e.g., celecoxib (Celebrex).
