Instructions

This panel includes testing for the presence of: Amphetamines, Methamphetamine, Phentermine, Alprazolam, Alpha-OH-Alprazolam, Butalbital, Barbiturates, Benzodiazepines, Clonazepam. 7- Amino clonazepam. Diazepam, Nordiazepam, Oxazepam, Lorazepam. Midazolam, Alpha-OH-Midazolam, Cocaine, Benzoylecgonine, m-OH-Benzoylecgonine, Coca ethylene, MDMA (ecstasy) Marijuana, Opiates, Phencyclidine (PCP). Phenobarbital, Temazepam, Zolpidem, Buprenorphine, Norbuprenorphine, Naloxone, Codeine, Dihydrocodeine, Fentanyl, Hydrocodone, Nor hydrocodone, Hydromorphone, Meperidine, Methadone, Methadone metabolite, 6-Acetylmorphine, Morphine, Methylphenidate, Oxycodone, Nor oxycodone, Oxymorphone, Tapentadol, Tramadol, N-desmethyltramadol, O-desmethyltramadol, Gabapentin, Mirtagynine.

Specimen Collection Criteria

Collect:  All meconium (blackish material) excreted until milk/formula-based stool (yellow-green) appears.

Note:  For maximum sensitivity, collect all meconium from first passage until transitional milk stool. For average sensitivity, collect four (4.0) grams. The laboratory will analyze amounts under four grams, but that sensitivity will be lost.

Physician Office/Draw Specimen Preparation

Maintain specimen refrigerated (2-8°C or 36-46°F) prior to transport.

Preparation for Courier Transport

Transport: 4.0 g meconium, refrigerated (2-8°C or 36-46°F). (Minimum: 2.0 g)

Rejection Criteria

  • Yellow stool specimens may be rejected.
  • Specimens not collected and processed as indicated.

In-Lab Processing

Maintain specimen refrigerated (2-8°C or 36-46°F) prior to transport.

Transport: 4.0 g meconium, refrigerated (2-8°C or 36-46°F). (Minimum: 2.0 g)

Storage

Specimen Stability for Testing:

Room Temperature (20-26°C or 68-78.8°F): 7 days
Refrigerated (2-8°C or 36-46°F): 3 weeks
Frozen (-20°C/-4°F or below): 1 year

Specimen Storage in Department Prior to Disposal:

Specimen retention time is determined by the policy of the reference laboratory. Contact the Send Outs Laboratory with any questions.

Test Methodology

Qualitative Liquid Chromatography / Tandem Mass Spectrophotometry.

Interpretation

Since meconium begins to form in the fetal gut about the fourth month, this procedure can provide up to a 20-week history of fetal drug exposure.

Detection of drugs in meconium is intended to reflect maternal drug use during approximately the last trimester of a full-term pregnancy. The pattern and frequency of drug(s) used by the mother cannot be determined by this test. A negative result does not exclude the possibility that a mother used drugs during pregnancy. Detection of drugs in meconium depends on the extent of maternal drug use, as well as drug stability, unique characteristics of drug deposition in meconium, and the performance of the analytical method. Drugs administered during labor and delivery may be detected. Detection of drugs in meconium does not insinuate impairment and may not affect outcomes for the infant. Interpretive questions should be directed to the laboratory.

Clinical Utility

Use to detect and document maternal drug use. Useful during approximately the last trimester of a full-term pregnancy.
