Collect (preferred specimen): One Gold-top SST tube.
Also acceptable: One plain Red-top tube.

Physician Office/Draw Specimen Preparation

Let specimen clot 30-60 minutes then centrifuge to separate serum from cells. Transfer serum to a plastic transport tube and refrigerate (2-8°C or 36-46°F).

Preparation for Courier Transport

Transport: 1.0 mL serum, refrigerated (2-8°C or 36-46°F). (Minimum: 0.5 mL)

Enzyme Immunoassay (EIA).

Interpretation

LKM (liver kidney microsome) autoantibodies are found in a subgroup of patients with ANA-negative autoimmune chronic active hepatitis. Antibody to hepatitis C virus can be found in about 50% of patients with low LKM autoantibody titers.

Antibodies to liver kidney microsome antigens (anti-LKM1) occur in a subset of patients with chronic autoimmune hepatitis. These patients have a high frequency of associated immunologic disorders and an absence of non-organ specific autoantibodies (smooth muscle antibodies). Most patients with type of chronic autoimmune hepatitis are children.

Clinical Utility

This assay is used to detect a clinically specific antibody in patients with autoimmune chronic hepatitis (type 2).
