Specimen Collection Criteria

Collect: One Gold-top SST tube. (Minimum Whole Blood: 4.0 mL)

Contact the Laboratory for acceptability of other tube types.

Physician Office/Draw Specimen Preparation

Let SST specimen clot 30-60 minutes then immediately centrifuge to separate serum from cells. Refrigerate (2-8°C or 36-46°F) the centrifuged SST tube within two hours of collection.

Preparation for Courier Transport

Transport: Centrifuged SST tube, refrigerated (2-8°C or 36-46°F). (Minimum: 1.0 mL)

Rejection Criteria

Red-top tubes with serum not separated from cells within two hours of collection. 

In-Lab Processing

Let SST specimens clot 30-60 minutes. Centrifuge SST tubes or Microtainers® to separate serum from cells. Deliver immediately to the appropriate testing station.

Males: 11-44 pg/mL.

Females:
Follicular: 21-251 pg/mL.
Mid cycle: 38-649 pg/mL.
Luteal: 21-312 pg/mL.
Post-menopausal: ≤28 pg/mL.

Test Methodology

Chemiluminescence Immunoassay.

Interpretation

  • High Estradiol serum values are seen with: Ovarian tumors, adrenal feminizing tumors, precocious puberty (female), liver disease, and male gynecomastia.
  • Low Estradiol serum values are seen with: Ovarian failure and oral contraceptives.
  • CAUTION: Interference by Fulvestrant
  • NOTE: This assay should NOT be used to assess estradiol levels for patients undergoing Fulvestrant treatment.  An alternate method, such as chromatography, should be used.  All analogues of the estradiol molecule have the potential to cause interference/cross reactivity with the Abbott ARCHITECT Estradiol assay. If estradiol testing is required on such a patient, please order Estrogens, Serum Fractionated. The sample will then be sent to Mayo Medical Labs for testing by LC-MS.  

Clinical Utility

Measurement of estradiol is used clinically in the investigation and management of fertility disorders, gynecomastia, estrogen-producing ovarian and testicular tumors and in hyperplasia of the adrenal cortex. Serum E2 is measured to determine the estrogen status of women, such as in some cases of amenorrhea, and as a guide to monitoring follicular development during ovulation induction and to avoid hyper-stimulation.
